A gentle, hands-off facial is fine 24 hours after botox. Anything involving heat, energy or muscle stimulation — microneedling, RF, HIFU, microcurrent, EMS — should wait a clear two weeks, because botulinum toxin takes roughly 10 to 14 days to bind fully to the nerve endings in the treated muscle.
That binding window is the whole reason the rule exists. Botulinum toxin works by blocking the release of acetylcholine at the nerve endings that tell a muscle to contract. The first effect shows at around three days, and the result peaks between one and two weeks after treatment — the interval set out in the UK Summary of Product Characteristics for Botox. Until it peaks, some of the product is still unbound and still mobile. Heat spreads it, firm pressure pushes it, and forced muscle contraction pulls it — and moving it even half a centimetre means it stops acting on the muscle your injector chose and starts acting on the one next door.
This is the part clients usually get backwards. The risk is not that your botox wears off faster; it is that it lands off-target and gives you a dropped brow, a heavy lid or an asymmetry you live with for three or four months. A placement problem, not a longevity one — and the same logic underpins every rule in botox aftercare.
Microneedling
Two weeks. Needling drags repeated pressure and traction across the exact area that was injected and drives controlled inflammation into the dermis — both can carry unbound product away from where it was placed. London clinics that publish their protocols, including Harley Street Skin Clinic, use the same 14-day interval.
RF microneedling
Two weeks, and this is the strictest of the group. It combines the mechanical pressure of needling with heat deliberately delivered into the dermis, and controlled heat in the injected tissue plane is the most efficient way there is to disperse product that has not finished binding.
HIFU and ultrasound
Two weeks. HIFU focuses thermal energy into the deeper layers — the same planes the toxin is settling in — and ultrasound-driven facials add surface heat and vibration on top. Both act below the skin — exactly where the problem would be.
Microcurrent and EMS
Two weeks. These work by making the muscle contract, which is precisely the action the botox was placed there to switch off — so during the binding window the treatment and the injection are pulling against each other.
Standard facials and facial massage
A gentle facial — cleanse, gentle exfoliation, a mask, extractions away from the injection points — is fine from 24 hours. Firm facial massage, lymphatic drainage over the treated area and anything face-down in a couch belong in the two-week group: sustained pressure moves liquid, which is the whole mechanism.
LED and red light
No real wait. LED is non-thermal — it does not heat the tissue, compress it or make the muscle fire — so it is the one thing on this list you can book without recalculating your dates. Most practitioners will still keep the panel off freshly injected skin for the first few hours.
If you're planning both
Sequence them the other way round. Book the energy treatment first — RF microneedling, HIFU, a microcurrent course — let any swelling settle, then have your botox. Nothing in the botox timeline is disturbed and you are not sitting on a two-week block.
If the botox has already happened, count 14 clear days and book the energy treatment for day 15 or later. Use the same window for your day-14 review with your injector: that appointment exists so an under-treated area or a small asymmetry can be topped up while the result is still fresh, and it reads far more accurately when nothing has been heated or needled over the top of it. Keeping the injection date, the areas treated and the review date written down is what makes the maths trivial next time — an injectable treatment tracker does that job.
Guidance, not medical advice. This is general information to help you get the most from a treatment — not a substitute for your practitioner. Botulinum toxin is a prescription-only medicine, and your qualified, regulated practitioner's specific aftercare always takes priority over anything here. If you have unusual pain, spreading weakness, drooping or signs of infection, contact your practitioner or a doctor. The NHS publishes advice on cosmetic procedures, including how to check who is treating you.
Frequently Asked Questions
Can I have microneedling after botox?
Yes, once you have left a clear two weeks. Needling before the toxin has finished binding risks moving it off-target, which shows up as an uneven brow or a heavy lid rather than as faded results.
How long after botox can I have a facial?
A gentle, low-pressure facial is fine after 24 hours. Anything with heat, steam, deep massage or lymphatic drainage over the treated area should wait two weeks.
Can I do RF microneedling after botox?
Wait two weeks. RF microneedling is the highest-risk combination on this list because it applies pressure and heat into the same tissue plane the toxin is settling in.
Is microcurrent safe after botox?
Not within the first two weeks. Microcurrent and EMS contract the muscle your botox was placed there to relax, so they pull directly against the treatment during the binding window. After 14 days it is generally fine — check with your injector.
